With your referral, please include:
- A copy of the front and back of your patient’s medical and prescription insurance cards
- Patient demographics and all clinical notes with an Onco360 order form (if applicable) for each prescription
Once we receive your referral, we will:
- Act as the advocate for your patient with you, their insurance company, and others involved in their treatment.
- Make sure they understand their medications, including dosage and any possible side effects. Clinicians and pharmacists are on call 24/7 if there are any issues.
- Deliver their prescription to them as quickly as possible. We know timing is crucial when it comes to cancer medications and even provide complimentary priority overnight shipping to a patient’s doorstep.
